'Systemic failure' in death of soldier who accidentally shot himself

The family of a soldier who died when his rifle went off during a training exercise fear a 'potential systemic failure' may have contributed to his death.

Lance Corporal Joe Spencer from Hampshire was killed on November 1, 2016, when his gun went off unexpectedly during a sniper training course in the Scottish Highlands.

He had been holding the rifle vertically, with the butt of the rifle on the toe cap of his boot and his chin resting on the top end, it was found.
